This protocol addresses a specific, select presentation of Paget's disease of the breast in which there is no associated palpable lump and no clinical or radiological evidence of an underlying breast tumor.
Paget's disease confined to the nipple — with no palpable lump in the breast and no clinical or radiological detectable breast tumor. This well-defined subset of patients requires a tailored management approach distinct from presentations with an associated mass.
The protocol for this scenario involves a surgical intervention. The specific indications, conditions, and sequencing that govern its application are detailed in the complete structured protocol.

There was no associated palpable lump in the breast.
This gives support to the view that selected patients with Paget's disease of the breast can be treated by radiotherapy as an alternative to radical surgery in selected patients with Paget's disease confined to the nipple, without clinical or radiological detectable breast tumor.
Mastectomy should be reserved for the few occasions, when relapse occurs.
