Treatment of Low-Grade Central Osteosarcoma, Parosteal Osteosarcoma, or Periosteal Osteosarcoma

Osteosarcoma encompasses several histological subtypes with distinct biological behaviours. Low-grade central osteosarcoma, parosteal osteosarcoma, and periosteal osteosarcoma represent a clinically important group with particular management considerations.

These subtypes — low-grade central osteosarcoma, parosteal osteosarcoma, and periosteal osteosarcoma — carry a lower metastatic potential compared to conventional high-grade osteosarcoma. Complete surgical removal is the central treatment strategy for this group.
Surgical resection is the cornerstone of management. Chemotherapy may be considered in select cases — specifically where resection histology reveals high-grade areas — though the full decision algorithm, sequence, and specifics of any systemic approach are outlined in the complete protocol.
Full regimen details, eligibility criteria, and sequencing are available via the structured protocol below.

Low-grade central, parosteal and periosteal osteosarcoma have lower metastatic potential, and complete surgical removal is recommended.

Resection histology may show high-grade areas associated with poorer outcomes.

Chemotherapy may be considered for these cases although evidence is limited.
