Treatment of Osteomalacia in Severe Secondary Hyperparathyroidism with Concomitant Calcium Malabsorption

Clinical scenario

This protocol is indicated for osteomalacia in patients with severe secondary hyperparathyroidism, particularly when one or more of the following conditions are also present:

Severe secondary hyperparathyroidism Concomitant calcium malabsorption Gastric bypass surgery Kidney failure
Treatment approach

In this clinical context, a combination of calcitriol and vitamin D is specifically preferred. The full regimen — including selection criteria, dosing, and monitoring guidance — is available in the structured protocol below.

Treatment goals

The aim is to maintain serum 25OHD above 30 ng/mL and bring PTH levels within the reference range. With effective therapy, clinical symptoms typically begin to improve within a few weeks; complete resolution may take several months.

The use of calcitriol along with vitamin D may be preferred in cases of severe secondary hyperparathyroidism, in patients with concomitant calcium malabsorption or gastric bypass surgery, and in those with kidney failure.

Target levels of serum 25OHD should be aimed at maintaining >30 ng/mL and PTH levels within the reference range.

With effective therapy, clinical symptoms begin to improve within a few weeks; however, complete resolution of symptoms may take several months.
