First-Line Treatment for Incurable, Recurrent or Metastatic Oral Cavity Carcinoma with PD-L1 Expression (CPS ≥1)

When oral cavity carcinoma has progressed to an incurable, recurrent or metastatic stage and the tumour qualifies on PD-L1 biomarker testing, first-line systemic therapy follows a specific evidence-based pathway.

This protocol is indicated for patients with incurable, recurrent or metastatic oral cavity carcinoma whose tumour and immune cells express PD-L1, confirmed by a combined positive score (CPS) of ≥1.

For PD-L1–positive disease, first-line therapy centres on a PD-1–targeting antibody, which may be used as monotherapy or as part of a combination regimen, with a structured maintenance phase to follow. The antibody pembrolizumab, which targets the PD-1 receptor, shall be used in patients with PD-L1-expressing tumor and immune cells (CPS ≥1) as first line monotherapy or in combination with platinum and 5-fluorouracil.

After 4-6 cycles of this combination, PD-L1 positive patients shall receive maintenance therapy until progression with pembrolizumab or PD-L1 negative patients with cetuximab.
