This protocol addresses onchocerciasis management in a specific and clinically important sub-population: patients with confirmed or suspected co-infection with Loa loa (loiasis), or those residing in a region where loiasis is co-endemic.
Co-infection with Loa loa — or living in an area where loiasis and onchocerciasis overlap — significantly alters the safety profile of standard onchocerciasis treatment. Patients with a high L. loa microfilarial load are at risk of severe adverse reactions from certain treatments, requiring a modified approach with close clinical supervision.
The approach in this scenario involves a specific oral agent administered under caution, with an alternative oral antibiotic therapy available for patients who have already developed signs of loiasis and in whom onchocerciasis has significant clinical impact. The choice between these options — and the conditions under which close supervision is required — is detailed in the full structured protocol.
The complete regimen specifies precise supervision requirements, the conditions governing each treatment option, and the full clinical algorithm for this co-infection scenario.
In case of co-infection with Loa loa or in regions where loiasis is co-endemic, ivermectin should be administered with caution (risk of severe adverse reactions in patients with high L. loa microfilarial load).
If the patient already has developed signs of loiasis and if onchocerciasis has a significant clinical impact, administer ivermectin under close supervision (see Loiasis) or use an alternative (doxycycline, as above).
