Treatment of Oligodendroglioma (CNS WHO Grade 3, IDH-Mutant, 1p19q-Codeleted) in Adults

This protocol covers first-line management of oligodendroglioma in adult patients whose tumour meets the molecularly defined criteria of CNS WHO grade 3 with a confirmed IDH mutation and 1p19q codeletion.

Clinical Scenario

The scenario: an adult patient with oligodendroglioma classified as CNS WHO grade 3, harbouring an IDH mutation and 1p19q codeletion. These molecular characteristics — IDH mutation and 1p19q codeletion — are integral to the current WHO CNS tumour classification and directly inform the treatment approach for this subgroup.

Treatment Approach

Evidence supports radiation therapy combined with chemotherapy as the recommended foundation for first-line treatment in this setting. The complete regimen — including specific agent selection and the conditions under which alternative approaches are considered — is detailed in the full evidence-based protocol.

Full regimen details, including the chemotherapy component and alternative options when tolerability is a concern, are available below.

  • People with oligodendroglioma, IDH-mutant, 1p19q co-deleted, CNS WHO grade 3 should be offered radiation therapy in combination with PCV (Evidence quality: Moderate; Strength of recommendation: Strong).
  • Temozolomide is a reasonable alternative to PCV when toxicity is a concern (Evidence quality: Low; Strength of recommendation: Conditional).
