Ocular sarcoidosis can involve the posterior segment of the eye, manifest as panuveitis, or both. When posterior uveitis or panuveitis is present, the clinical picture typically demands a systemic rather than purely local approach.
This protocol is for patients with ocular sarcoidosis presenting with posterior uveitis or panuveitis. For this pattern of involvement, systemic therapy is usually used.
Management may involve adding or switching to systemic agents — including biologic anti-TNF therapy — as part of the regimen. The complete sequence, agent selection, and supporting options are set out in the full protocol.
For posterior uveitis or panuveitis, systemic therapy is usually used.
Recent experience suggests that the biologic agents infliximab or adalimumab, both anti-TNF monoclonal antibodies, are also effective.
In uveitis in general – including uveitis related to sarcoidosis – either infliximab or adalimumab has been useful in refractory cases.
