Treatment of Non-bullous Impetigo When the Patient Is Systemically Unwell or at High Risk of Complications

Most presentations of non-bullous impetigo can be managed locally, but when a patient is systemically unwell or carries factors that heighten the risk of complications, systemic treatment is indicated. The management approach in this scenario differs from routine cases.

This protocol addresses non-bullous impetigo in patients who are systemically unwell or who are at high risk of complications. In these circumstances, a more assertive treatment strategy is warranted.

A short course of an oral antibiotic is recommended. The choice of agent is guided by individual patient factors — including allergy status and pregnancy — with clearly defined alternatives when the first-line option is unsuitable. Cure or measurable improvement of non-bullous impetigo by the end of the treatment course. A 5-day course is usually appropriate.
