Treatment of Nasopharyngeal Cancer in Local or Regional Recurrence Amenable to Salvage Surgery or Re-irradiation

Recurrence of nasopharyngeal carcinoma — whether at the primary site or in regional lymph nodes — poses a significant clinical challenge. In selected patients, active salvage approaches remain feasible and are the focus of structured evidence-based management.

Clinical Scenario

This protocol applies to patients with local or regional recurrence of nasopharyngeal carcinoma who are amenable to salvage surgery or re-irradiation. A critical determinant of candidacy is whether the local recurrence invades the carotid artery or extends intracranially — patients without these features represent a subgroup for whom surgical salvage may be appropriate.

Salvage Treatment Approach

Management involves surgical and/or re-irradiation-based strategies, selected according to the recurrence pattern and anatomical extent. Both local recurrences and lymphatic neck recurrences have distinct therapeutic pathways within the evidence base. The complete structured regimen — including specific surgical approaches, radiotherapy modalities, sequencing, and the role of concurrent systemic treatment — is available via the link below.
