Treatment of Nasal Cavity Cancer in Malignant Melanoma or Sarcoma

Nasal cavity cancers arising as malignant melanoma or sarcoma represent a distinct and uncommon clinical scenario. These histological subtypes are managed differently from the more common epithelial carcinomas of the nasal cavity and paranasal sinuses.

This protocol addresses nasal cavity cancer where the underlying pathology is malignant melanoma or sarcoma — including mucosal melanoma and subtypes such as rhabdomyosarcoma. Malignant melanoma presents in less than 1% of neoplasms in this region. Lymphomas, sarcomas, and mucosal melanomas of the paranasal sinuses and nasal cavity are not staged using the standard staging system for this region, and their management reflects distinct histology-specific pathways.

When surgically feasible, the approach centres on excision. In cases where surgery is not possible, radiation therapy is among the options considered. For certain sarcoma subtypes, a combined modality approach involving surgery, radiation, and systemic therapy is recommended. The full algorithm — including specific agent selection, sequencing, and stage-dependent considerations — is available in the complete protocol.
