Treatment of Narcolepsy Type 2 with Excessive Daytime Sleepiness (No Cataplexy)

This protocol applies to patients with narcolepsy type 2 (NT2) — presenting with excessive daytime sleepiness (EDS) in the absence of cataplexy.

There are two primary classifications of narcolepsy: narcolepsy type 1 (NT1) and narcolepsy type 2 (NT2). The hallmark symptom is excessive daytime sleepiness across both types, but cataplexy is specific to NT1 and is not present in this scenario.

Evidence-based guidelines strongly recommend wake-promoting agents as the primary pharmacological intervention for EDS in narcolepsy type 2. Several agent options are supported, with the full protocol detailing the structured selection and titration approach.

Complete regimen, agent selection criteria, and dosing algorithm available via the full protocol.
Target: ESS score ≤ 10 & meaningful reduction in daytime sleepiness

There are 2 primary classifications of narcolepsy, narcolepsy type 1 (NT1) and narcolepsy type 2 (NT2).

The hallmark symptom is excessive daytime sleepiness (EDS), but only patients with NT1 experience cataplexy.

For the management of EDS, the American Academy of Sleep Medicine (AASM) guidelines strongly support use of modafinil, pitolisant, sodium oxybate, and solriamfetol.

Screening typically begins in a clinic with the administration of the Epworth Sleepiness Scale (ESS) on which scores greater than 10 indicate the need for additional evaluation.
