Treatment of Newly Diagnosed Multiple Myeloma in Patients Under 70 With No Comorbidities
This protocol addresses fit patients with newly diagnosed multiple myeloma (NDMM) who are under 70 years of age, have no significant comorbidities, and are candidates for intensive therapy.
The patient is newly diagnosed with multiple myeloma, is under 70 years of age, has no significant comorbidities, and is eligible for high-dose therapy (HDT) and autologous stem cell transplantation (ASCT). This fit profile makes the patient a candidate for intensive, guideline-recommended treatment.
The recommended strategy for this patient profile involves multi-drug combination induction therapy, followed by high-dose conditioning and autologous stem cell transplantation, with maintenance therapy continuing after transplant.

- For fit NDMM patients, aged <70 years, without comorbidities, induction followed by high-dose therapy (HDT) with autologous stem cell transplantation (ASCT) and lenalidomide maintenance is the recommended treatment.
- Maintenance with lenalidomide is considered the standard of care for all MM patients post-ASCT [I, A]; bortezomib may be considered for patients with high-risk disease [II, B].
