Treatment of MEN2B with Advanced or Metastatic Medullary Thyroid Carcinoma

This protocol addresses patients with Multiple endocrine neoplasia type 2B who present with advanced, locally advanced, or metastatic medullary thyroid carcinoma — specifically those with significant tumor burden and symptomatic or progressive disease as defined by RECIST criteria.

Clinical scenario: Advanced or metastatic medullary thyroid carcinoma in the setting of MEN2B, with significant tumor burden. Disease is symptomatic or radiologically progressive per RECIST, warranting systemic treatment. In patients meeting these criteria, treatment with agents targeting both RET and VEGFR tyrosine kinases should be considered as systemic therapy.

Treatment approach (partial overview)

The regimen involves combination cytotoxic chemotherapy; specific doublet combinations are incorporated into the full protocol. The complete sequence, agent selection, and all dosing details are available in the structured evidence-based regimen.

In patients with significant tumor burden and symptomatic or progressive metastatic disease according to RECIST treatment with TKIs targeting both RET and VEGFR tyrosine kinases should be considered as systemic therapy.

The most effective regimens are combination therapy with doxorubicin and another agent, or 5-fluorouracil and dacarbazine.
