Morton Neuroma When Corticosteroid Injection Fails to Relieve Forefoot Pain

Clinical scenario

This protocol applies to patients with Morton neuroma who have completed a course of corticosteroid injection therapy targeting the affected intermetatarsal space and have not achieved adequate reduction of forefoot pain.

Prior treatment — goal not reached
Previous line: Corticosteroid injection

Corticosteroid injection into the affected intermetatarsal space — the mainstay of injection treatment — was used to reduce inflammation surrounding the neuroma and relieve pain and local pressure effects. The expected goal of reducing forefoot pain was not achieved, triggering escalation to this next-line protocol.

Next-step approach

When injection therapy has not achieved the expected forefoot pain relief, surgical excision of the neuroma is among the interventions this protocol covers — the complete approach is in the structured regimen below.
