Treatment of Monoclonal Gammopathy of Clinical Significance with Organ Damage Involving Skin, Peripheral Nerves, or Cardiac Muscle

In monoclonal gammopathy of clinical significance (MGCS), target organs including the skin, peripheral nerves, and cardiac muscle can be involved. The central aim of management in this setting is to prevent further tissue damage to these affected organs.

This protocol applies to patients with monoclonal gammopathy of clinical significance in whom organ damage involves one or more of the following: skin, peripheral nerves, or cardiac muscle. Preventing ongoing damage to these target organs is the treatment priority.

Management requires systemic therapy directed at controlling the underlying B-cell clone. The decision to initiate treatment should be made by a multi-disciplinary team (MDT) with appropriate sub-specialty input.

The complete regimen — specific agent selection, sequencing, and monitoring parameters — is detailed in the full protocol below.

Other target organs, such as the skin, peripheral nerves and cardiac muscle, can be involved in MGCS (see Table 3), and the principle of management to prevent further tissue damage requires the use of systemic chemotherapy to control the underlying B-cell clone.

The decision to instigate such treatment should be made by a multi-disciplinary team (MDT) with suitable sub-specialty representation.
