Treatment of Moderate to Severe Hypoglycemia in a Conscious, Orientated Patient Able to Swallow (Blood Glucose < 4.0 mmol/L)
This first-line protocol applies when a patient with blood glucose below 4.0 mmol/L is conscious, orientated, and able to swallow — conditions that allow safe oral treatment to be initiated immediately.

The patient is conscious and orientated, has an intact swallow, and presents with a capillary blood glucose below 4.0 mmol/L, meeting the threshold for moderate to severe hypoglycemia requiring prompt intervention.

Treatment Goal
Capillary blood glucose above 4.0 mmol/L when remeasured 10–15 minutes after treatment.

- In people who are conscious and able to swallow, 15-20g of rapid-acting carbohydrate is the treatment of choice (see algorithm A).
- Give 15-20g rapid-acting carbohydrate of the person with diabetes in hospital's choice where possible.
- Once blood glucose is above 4.0mmol/L and the person has recovered, give a long-acting carbohydrate snack (20g) of their choice where possible, taking into consideration any specific dietary requirements.
- Repeat capillary blood glucose measurement 10-15 minutes later. If it is still less than 4.0mmol/L, repeat step 3 (no more than 3 treatments in total). If it is above 4.0mmol/L then go to step 7.
