Treatment of Chronic Severe Primary Mitral Regurgitation

This protocol addresses the management of patients with chronic severe primary mitral regurgitation (PMR) — a structural valve disease in which the primary pathology lies within the mitral valve apparatus itself, producing significant regurgitation that requires clinical assessment and, where indicated, surgical intervention.

When surgery is indicated, the structured protocol addresses valve intervention options — including, in appropriate cases, a surgical approach involving valve replacement technique with specific anatomical considerations — with further steps, indications, and sequencing available in the full protocol.

  • Indications for surgery in patients with chronic asymptomatic and symptomatic PMR are summarized in Recommendation Table 6 and Figure 11.
  • When MV repair is not feasible, valve replacement with preservation of the subvalvular apparatus should be performed.
