Minimal Change Disease: Treatment When Glucocorticoids Are Contraindicated
Standard first-line management of minimal change disease centres on glucocorticoid therapy. When a patient carries an absolute or relative contraindication to these agents, an alternative first-line strategy is needed.
Clinical Scenario
Minimal change disease in a patient with an absolute or relative contraindication to glucocorticoids — circumstances in which standard steroid-based induction cannot safely proceed and a steroid-sparing approach must be chosen from the outset.
Treatment Approach (Overview)
Initial management involves an alternative immunosuppressive agent. Calcineurin inhibitors are among the options considered in this setting — the complete selection of agents and the clinical decision framework are covered in the full structured protocol.

- For patients in whom glucocorticoids may be relatively contraindicated, consider initial therapy with cyclophosphamide, a CNI, or MMF.
- In patients with absolute or relative contraindication to steroids, we agree alternative immunosuppressive agents such as CNI, Cyclophosphamide, MMF or Rituximab should be considered.
