Treatment of Microscopic Colitis with Coexisting Bile Acid Diarrhoea

Bile acid diarrhoea is a recognised comorbidity in patients with microscopic colitis (MC). When both conditions coexist, the bile acid component requires specific therapeutic attention alongside standard MC care.

Clinical Scenario

Patients with microscopic colitis who have coexisting bile acid diarrhoea — a combination that warrants a distinct, targeted treatment approach.

Treatment Approach

Management in this setting involves bile acid binders as the primary therapeutic class. The complete protocol — including agent selection, structured sequencing, and full regimen details — is available via the link below.

Treatment Goal

In patients with MC and bile acid diarrhoea we suggest treatment with bile acid binders.

A large, prospective cohort study demonstrated that bile acid diarrhoea diagnosed with SeHCAT coexists with MC with an estimated prevalence of approximately 14%, and 84 of 167 patients treated with cholestyramine reported subjective cessation of diarrhoea.
