Treatment of Primary Immune Complex MPGN in Adults Aged 18 and Older — Native and Post-Transplant Kidney

This protocol is for adults aged 18 years or older with a diagnosis of primary immune complex membranoproliferative glomerulonephritis (IC-MPGN) — applicable to both native kidney and post-transplant kidney presentations.

Management is built on general supportive care targeting the principal manifestations of glomerular disease — oedema, proteinuria, and hypertension. The approach incorporates lifestyle measures alongside pharmacological strategies aimed at blood pressure control and proteinuria reduction, as well as several additional interventions. The complete structured regimen, including the full range of options and their sequence, is available in the protocol.

References

  • General supportive care for all patients with glomerular diseases who have edema, proteinuria and/or hypertension
  • RAASi (e.g. ACEi or ARBs) and potentially other antiproteinuric agents
  • Alternative anti-hypertensives (e.g. CCBs)
  • Treatments have focused on nonspecific approaches of key importance in many kidney diseases such as optimization of blood pressure and proteinuria control.
