This protocol applies to patients with metastatic medullary thyroid carcinoma who have developed Cushing's syndrome due to ectopic production of ACTH or CRH. These patients are often markedly debilitated and warrant active management despite their poor prognosis.
Medical therapy to control cortisol excess — including agents such as ketoconazole, mifepristone, aminoglutethimide, metyrapone, or mitotane — did not achieve normalization of elevated serum cortisol levels. This protocol represents the next step when the medical approach is refractory.
When medical therapy has failed to normalize cortisol levels, the protocol specifies a surgical intervention targeting the adrenal glands. The full decision criteria, patient selection, and procedural details are available in the complete protocol.
