High-risk mantle cell lymphoma encompasses a subset of patients whose tumour biology demands a differentiated first-line strategy. Risk stratification based on molecular and morphological features is central to selecting the appropriate regimen.
Patients with high-risk MCL are typically characterised by a high-risk MIPI score, high Ki67, blastoid or pleomorphic morphology, and/or a TP53 mutation or p53 overexpression. This combination of features defines a population for whom standard approaches may be insufficient and tailored first-line treatment selection is essential.
