Treatment of Advanced-Stage Mantle Cell Lymphoma in Fit Younger Patients (Typically Up to 65 Years)

This protocol covers the initial management of fit younger patients — generally up to age 65 — presenting with advanced-stage mantle cell lymphoma who are candidates for intensive, dose-intense therapy.

The target population is fit younger patients with advanced-stage mantle cell lymphoma who are eligible for intensive induction strategies. Guidance also applies to fit patients beyond 65 years when overall fitness supports this approach.

Treatment Approach (partial overview)

For this population, current evidence supports intensive alternating induction followed by extended maintenance therapy, with decisions on additional consolidation personalised to depth of response after induction.

Treatment Goal

The primary aim is complete response with molecular remission, assessed by an NGS-based assay after induction.

Most trials in this population included patients up to 65 years, but we advise fit patients >65 years also to be treated according to the following (Figure 1).
Fit younger patients should be treated with R‑CHOP‑Ibrutinib/R‑DHAP or R‑DHAOx induction, followed by 2 years of ibrutinib and 3 years of rituximab maintenance [I, A].
Consider discussing ASCT in selected patients if high‑risk features are present [II, C].
In patients in CR with molecular remission by the NGS‑based assay postinduction, ASCT can be omitted [II, A].
