Treatment of Disseminated Gastric MALT Lymphoma with Lymph Node Involvement

Gastric MALT lymphoma most often presents as localised disease. In the uncommon cases where it has disseminated or lymph node involvement is identified, local strategies are insufficient and a systemic approach managed by haematology is required, with the aim of achieving complete remission.

Clinical Situation

Disseminated gastric MALT lymphoma, or disease with lymph node involvement — scenarios in which systemic intervention is indicated rather than conservative or local measures alone.

Treatment Approach

Systemic immunochemotherapy is the established strategy in this setting. Rituximab-based combination regimens are central to treatment, and evidence supports their use as the basis for achieving complete remission. The complete regimen selection and protocol are detailed in the full resource below.

  • In very rare cases of disseminated diseases or with lymph node involvement, a systemic treatment based on immunochemotherapy may be proposed, usually led by Hematologists.
  • The association of Rituximab and chlorambucil appeared more effective than chlorambucil alone in a phase III study.
  • More recently, the association of rituximab with bendamustine has been reported as giving a complete remission already after three cycles.
