Lip Cancer Stage III–IV with Positive Surgical Margins or Extracapsular Spread — Treatment in Patients Under 71

This protocol covers operable lip cancer at stage III or IV where surgery has left positive margins and/or nodal metastasis shows extracapsular spread, in patients who are fit for chemotherapy and aged under 71.

Clinical Scenario

Lip cancer, stage III or IV, operable — patient is fit for chemotherapy, under 71 years of age, and presents with positive surgical margins and/or extracapsular spread of nodal metastasis. These high-risk pathological features define the need for additional structured management.

Treatment Approach (partial)

In fit patients under the age of 71, adjuvant radiochemotherapy up to 66 Gy with concurrent platinum-based chemotherapy should be considered for those with positive surgical margins and/or ECS.

Patients with locally recurrent disease should be fully restaged and assessed for consideration of curative treatment in the form of repeat surgery, possible EBRT or brachytherapy if available.
