Treatment of Vasculogenic Erectile Dysfunction When Intracavernous Injection Therapy Has Not Achieved Satisfactory Rigidity

This protocol applies to men with vasculogenic erectile dysfunction whose prior intracavernous injection therapy did not achieve a rigid erectile response sufficient for satisfactory intercourse.

Clinical scenario

Vasculogenic erectile dysfunction is the specific subtype of male erectile dysfunction addressed by this protocol.

Previous treatment line — escalation trigger

The previous treatment line included intracavernous alprostadil, Trimix, or Invicorp. Escalation to this protocol is triggered when those therapies did not achieve a rigid erectile response sufficient for satisfactory intercourse.

Next-step approach & clinical goal

When intracavernous injection therapy has not produced the required outcome, a surgical implantation procedure forms the basis of the next management step. The clinical goal is restoration of penile erection sufficient for satisfactory intercourse.

  • Implant a penile prosthesis if other treatments fail or depending upon patient preference.
  • The surgical implantation of a penile prosthesis may be considered in patients who i) are not suitable for different pharmacotherapies or prefer a definitive therapy; and, ii) do not respond to other treatment modalities.
  • Efficacy is defined as an erection, with rigidity, sufficient for satisfactory intercourse.
