Treatment of Lupus Profundus in Pregnancy or Breastfeeding

Active lupus profundus during pregnancy or breastfeeding presents a specific clinical challenge: disease control must be achieved while maintaining safety for the fetus or nursing infant. Treatment selection is therefore guided by compatibility with these states.

This protocol applies to patients with active lupus profundus who are pregnant or breastfeeding. Both situations require careful evaluation of therapeutic options to balance effective disease management with the safety considerations specific to each state.

Treatment approach

In active disease during pregnancy or breastfeeding, we recommend HCQ as first line treatment for CLE at usual dosage.

We recommend that systemic corticosteroids (prednisone and methylprednisolone) should be given in a dosage of not more than 10 - 15 mg per day during pregnancy or breastfeeding.
