This page addresses the clinical management of patients who present with limited-stage small cell lung cancer at clinical stage IIB through IIIC and whose ECOG performance status is 3 or 4 as a direct consequence of the disease itself.
The defining features of this population are limited-stage SCLC (clinical stage IIB–IIIC) combined with a poor ECOG performance status of 3–4 attributable to the tumour. The distinction that the performance impairment is caused by SCLC — rather than unrelated comorbidities — is clinically significant: it informs both the rationale for treatment and the expected trajectory of performance recovery with response.
The recommended strategy involves systemic cytotoxic chemotherapy, which may be combined with radiotherapy administered concurrently or sequentially.
