Treatment of Advanced Nonsquamous NSCLC with EGFR Exon 20 Insertion Mutation

Clinical Scenario

EGFR Exon 20 Insertion

This protocol applies to adults with advanced or metastatic nonsquamous non-small cell lung cancer (NSCLC) who have a confirmed EGFR exon 20 insertion mutation. This mutation subtype is among the third most common group of EGFR mutations, occurring in approximately 2% of patients with NSCLC and in 4–12% of patients with EGFR mutations overall.

First-Line Treatment Approach

First-line management involves a combination regimen pairing a targeted bispecific antibody with platinum-based chemotherapy — a preferred, category 1 recommended strategy for this mutation-defined population.

Full regimen details, sequencing, and dosing are available in the complete protocol below.

References

EGFR exon 20 insertion mutations are the third most common group of EGFR mutations; these heterogeneous mutations occur in approximately 2% of patients with NSCLC and 4% to 12% of patients with EGFR mutations.

The Panel recommends amivantamab-vmjw in combination with carboplatin and pemetrexed as a category 1 and preferred first-line treatment option for patients with advanced or metastatic nonsquamous EGFR exon 20 insertion mutation-positive NSCLC.
