Treatment of Localized Dedifferentiated Liposarcoma in the Extremity or Trunk

Dedifferentiated liposarcoma (DDLPS) arising in the extremity or trunk follows a treatment framework comparable to other soft tissue sarcomas at these sites, with limb preservation as the central surgical objective.

Clinical Scenario

This protocol addresses patients with localized dedifferentiated liposarcoma confined to the extremity or trunk. In this anatomical setting, limb-sparing surgery is considered the standard of care, with the goal of achieving adequate oncologic margins while preserving limb function.

Treatment Approach

Management is built around limb-sparing oncologic resection of the DDLPS with oncologically sound surgical margins. For high-grade tumors, radiation therapy — delivered either before or after surgery — is an important component of local control. In patients considered to be at elevated risk for recurrence, perioperative chemotherapy may also be part of the approach.
