Treatment of Advanced or Metastatic Dedifferentiated Liposarcoma

Dedifferentiated liposarcoma (DDLPS) in the advanced or metastatic setting is an aggressive presentation with limited systemic therapy options. Selecting an appropriate treatment sequence is essential.

Clinical scenario
Advanced or metastatic dedifferentiated liposarcoma. Anthracycline-based therapy — either as a single agent or as part of a combination regimen — is considered the front-line standard for metastatic DDLPS.
Treatment approach (partial summary)

Following approved chemotherapy, current guidelines support the incorporation of immune checkpoint inhibitor-based therapy in this setting.

Anthracycline-based therapy, either as a single agent (doxorubicin or epirubicin) or as part of a combination (doxorubicin or epirubicin in combination with ifosfamide), is considered the front-line standard for metastatic DDLPS.

Pembrolizumab or nivolumab (with or without ipilimumab) are listed in NCCN guidelines for subsequent lines of therapy for advanced or metastatic disease.

Given limited systemic therapy options, we typically incorporate immune checkpoint inhibitors after approved chemotherapies.
