Clinical Scenario

This protocol addresses first-line management of Linear IgA bullous dermatosis (LAD) in both adult and paediatric patients. The approach applies across this patient population at the initial treatment stage.

Treatment Approach

First-line management is centred on a specific oral agent, used alone or combined with a potent topical corticosteroid based on clinical response. Dosing is weight-adjusted and titrated over the treatment course.

Treatment Goal

The primary endpoint is conversion of direct immunofluorescence (DIF) to negative at a previously positive lesion site, confirming sustained remission.

Dapsone is recommended as first-line treatment for LAD in all patients with normal G6PD activity.
It is recommended to use a starting dose of dapsone of 25-50 mg/day in adults, 0.5-1 mg/kg in children.
It is recommended to reach a daily dose of dapsone of 0.5-2 mg/kg in adults.
Potent topical corticosteroids such as clobetasol propionate are generally preferred.
Treatment discontinuation may be considered when negative DIF is obtained from a previously positive lesion site.
