Genital Lichen Sclerosus in Boys Under 18: What to Do After Topical Corticosteroid Failure

This protocol covers boys under 18 years of age with genital lichen sclerosus in whom initial topical corticosteroid treatment has not achieved the expected clinical response.

Clinical Scenario

Male patient, age under 18 years, with genital lichen sclerosus. Previously treated with ultrapotent or potent topical corticosteroids, co-treated with topical emollients.

Previous Treatment — Failure Condition

First-line treatment with ultrapotent or potent topical corticosteroids (co-treated with topical emollients) did not achieve the required goal: improvement of the clinical grade of phimosis. This protocol defines the structured next step following that failure.

Next-Line Approach (Partial Overview)

When topical corticosteroids are insufficient or contraindicated, a topical calcineurin inhibitor may be considered as a second-choice or additional treatment option. The complete protocol specifies the agent selection and full regimen details.

Treatment Goal

Suppression of symptoms.
