Genital Lichen Sclerosus in Women — Next Step When Topical Corticosteroids Have Not Achieved Remission

This protocol applies to women aged 18 or older with genital lichen sclerosus who are not pregnant and who have completed a first-line topical corticosteroid course without reaching the expected treatment goals.

Female patient, age 18 or older — genital lichen sclerosus, not currently pregnant.

A completed course of ultrapotent or potent topical corticosteroids (clobetasol propionate or mometasone furoate) did not achieve the expected targets: complete remission of symptoms, healing of fissures and erosions, near-complete resolution of hyperkeratosis, or adequate softening of sclerosis. This protocol defines the next step.

When topical corticosteroids are insufficient, the protocol calls for a topical calcineurin inhibitor as a second-choice or adjunctive option; for hyperkeratotic lesions that remain resistant to topical steroids — and in which malignancy has been excluded — an intralesional corticosteroid approach may also be considered. Treatment goal: Suppression of symptoms — including pruritus, burning, and dyspareunia — assessed at 12 weeks.
