What Is the First-Line Treatment for Genital Lichen Sclerosus in Women Aged 18 or Older?

This protocol covers adult women (18 years and older) with genital lichen sclerosus who are not pregnant. A defined evidence-based first-line regimen applies to this population.

Female patient, age 18 or older, with confirmed genital lichen sclerosus. Not currently pregnant. Because pregnancy and lactation restrict treatment options, this protocol applies specifically to non-pregnant women.
The recommended first-line approach centres on ultrapotent or potent topical corticosteroids, used alongside topical emollients during the active phase. The specific agents, application schedule, and stepwise structure are detailed in the full protocol.

We recommend ultrapotent or potent topical corticosteroids in women with genital lichen sclerosus.

We recommend co-treatment with topical emollients during standard therapy in women with genital lichen sclerosus.

All other treatments are not recommended during pregnancy and lactation.

About 60%–70% of LS patients achieve complete remission of their symptoms after a 3-month course of clobetasol propionate 0.05%, usually once daily application.

Healing of fissures and erosions. Hyperkeratosis should remit almost completely. Sclerosis should soften: improvement of phimosis.
