Leukoplakia of Vagina in Vulvar Lichen Sclerosus When Calcineurin Inhibitor Treatment Has Failed

This protocol addresses patients with leukoplakia of the vagina in the context of vulvar lichen sclerosus (VLS) who have not achieved adequate symptom control on prior therapy, and in whom topical calcineurin inhibitor treatment has been exhausted.

Two non-neoplastic epithelial disorders of the vulva — vulvar lichen sclerosus (VLS) and squamous cell hyperplasia of the vulva — are generally referred to as vulvar leukoplakia. This protocol is specific to patients with VLS as the underlying condition.
Previous Line — Why This Protocol Is Needed The patient has previously received topical calcineurin inhibitors (tacrolimus or pimecrolimus) — used as second-line therapy for VLS refractory to corticosteroids — and that treatment failed to achieve the key therapeutic goal: resolution of genital itching. Escalation to a third-line approach is now indicated.
