This protocol addresses leukemia cutis occurring in the setting of hairy cell leukemia (HCL) — a subtype of B-cell indolent lymphoid leukemia. Direct cutaneous involvement by HCL requires specific management, particularly when initial systemic therapy has not achieved skin clearance.
This protocol is indicated when first-line purine analog therapy — cladribine or pentostatin — has failed to achieve disappearance of cutaneous infiltrates and complete resolution of skin lesions. Non-achievement of these skin-clearance targets is the trigger for escalation to the next treatment line.
In this refractory or relapsed setting, a targeted BRAF inhibitor-based approach is among the options considered — full agent selection, sequencing, and management details are available only in the complete protocol.
