Treatment of Leukemia Cutis in Chronic Lymphocytic Leukemia

Leukemia cutis (LC) represents direct infiltration of the skin by leukemic cells. When it occurs in the setting of chronic lymphocytic leukemia (CLL), it poses a distinct clinical challenge that calls for a targeted management approach focused on local disease control.

This protocol addresses histologically confirmed leukemia cutis arising in a patient with chronic lymphocytic leukemia. LC is a rare complication in CLL and most frequently occurs in patients with Richter transformation.

Management in this setting centers on local therapy directed at the cutaneous lesions. Radiation-based approaches represent one of the options for achieving local control of the skin involvement.

The complete evidence-based regimen — including the full range of local treatment modalities, selection criteria, and sequencing — is available in the structured protocol below.

However, histologically confirmed LC is a rare event in CLL and most frequently occurs in patients with Richter transformation.

However, local therapy, including radiation therapy, excision, intralesional steroids, ultraviolet light B, and electrochemotherapy can be useful in local control of the skin lesions.
