Treatment of Kidney Stone Disease in Children Under 18 Years

Clinical Scenario

This protocol applies to children and adolescents under 18 years presenting with urinary stone disease. The indications for stone removal procedures in this age group are similar to those in adults, and the approach depends on stone size, location, and suitability for specific interventions.

Management Approach

Management centres on active stone removal. The specific technique — which may involve shock wave lithotripsy, ureteroscopy, or percutaneous nephrolithotomy — is selected based on stone characteristics. The full protocol specifies which intervention is indicated under which circumstances.

Complete decision criteria, including size thresholds and preferred sequencing, are available in the structured protocol.

References

  1. In children, the indications for SWL, URS and PCNL are similar to those in adults.
  2. Offer children with single ureteral stones less than 10mm shock wave lithotripsy (SWL) if localisation is possible or ureteroscopy as first-line option.
  3. Ureteroscopy is a feasible alternative for ureteral stones not amenable to SWL.
  4. Offer children with renal stones with a diameter of up to 20mm (±300mm²) SWL.
  5. Offer children with renal pelvic or calyceal stones with a diameter >20mm (±300mm²) percutaneous nephrolithotomy.
  6. Mini-PCNL is safe and effective in children.
