Treatment of Juvenile Myoclonic Epilepsy with Myoclonic Seizures in Adults

Adults 16 years of age and older

This protocol covers juvenile myoclonic epilepsy in adults aged 16 and older who present with myoclonic seizures. Achieving and maintaining seizure control in this population requires a structured, evidence-based approach.

Clinical Scenario

Adults 16 years of age and older with myoclonic seizures represent a population for whom a clear treatment pathway is essential. Up to 30% of patients with epilepsy can develop medically refractory disease, making a defined escalation strategy a critical part of management.

Treatment Goals

Approach

For adults with myoclonic seizures that remain medically refractory, the protocol identifies specific non-pharmacologic interventions — the full regimen details which approaches apply, how they are selected, and the clinical criteria that guide their use.

  4. For patients with seizures that are not controlled with these agents, alternative treatments include surgical resection of the seizure focus, ketogenic diets, vagus nerve stimulators, and implantable brain neurostimulators.
  5. Vagus nerve stimulation may increase seizure-free time in patients with medically refractory epilepsy who are not candidates for surgery or in whom surgery has been ineffective.
  6. Surgical resection of the seizure focus in appropriately selected patients often results in decreased frequency or elimination of seizures with improvement in quality of life.
