Treatment of Juvenile Enthesitis-Related Arthritis with Active Enthesitis (Tenderness and/or Swelling of the Entheses)

Clinical scenario

This protocol applies to children and adolescents with juvenile enthesitis-related arthritis who have active enthesitis — tenderness and/or swelling at the entheses (tendon-to-bone insertion sites) — determined by the treating provider to require medical treatment. Patients in this group most commonly fall within the ILAR categories of enthesitis-related arthritis, psoriatic arthritis, or undifferentiated arthritis, though any ILAR JIA category may be represented.

Treatment approach (partial overview)

For children and adolescents with JIA and active enthesitis, a specific class of anti-inflammatory medication is the strongly recommended first-line treatment over no pharmacological treatment.

The complete structured regimen — including specific agents, sequencing, and further management steps — is available via the full protocol.

This group is intended to include patients with enthesitis (inflammation at tendon-to-bone insertion sites) who will also most likely be from the ILAR categories of enthesitis-related arthritis, psoriatic arthritis, and undifferentiated arthritis but may include patients from any of the ILAR JIA categories.

For the purposes of this guideline, active enthesitis is tenderness and/or swelling of the entheses determined to require medical treatment per the treating provider.

In children and adolescents with JIA and active enthesitis, NSAID treatment is strongly recommended over no treatment with an NSAID.
