Treatment of Iron-Deficiency Anemia in Gastric Antral Vascular Ectasia (GAVE)

Iron-deficiency anemia arising in the setting of gastric antral vascular ectasia (GAVE) presents a specific clinical challenge. Both conditions must be addressed together in a coordinated management plan.

This protocol applies to patients with iron-deficiency anemia occurring secondary to gastric antral vascular ectasia. Because GAVE is an ongoing source of blood loss, iron repletion alone may be insufficient in individuals with an inadequate response to iron replacement.
All patients should receive iron repletion. The route — oral or intravenous — is guided by the severity of iron depletion and individual tolerance.

The complete protocol specifies the full selection criteria, escalation pathway, and additional interventional options.

  • All patients with IDA and GAVE should receive iron repletion, which may be oral or IV depending on the severity of iron depletion and tolerance.
  • In individuals with iron-deficiency anemia secondary to gastric antral vascular ectasia who have an inadequate response to iron replacement, consider endoscopic therapy with endoscopic band ligation or thermal methods such as argon plasma coagulation.
