This protocol addresses interstitial keratitis arising from a bacterial cause, specifically in cases where syphilitic aetiology has been excluded. Management combines local ocular therapy with treatment directed at the systemic underlying infection.
Bacteria-mediated interstitial keratitis — excluding syphilitic interstitial keratitis — encompasses a distinct group of cases in which the corneal inflammation is driven by a systemic bacterial process other than syphilis. Addressing the underlying systemic infection is central to achieving resolution; local therapy alone is insufficient.
Corticosteroids have a role in the local therapy of most forms of bacteria-mediated interstitial keratitis, but all will require specific systemic antibiotics to cure the underlying disease.
With the exception of syphilis, all of the other forms of bacteria-mediated interstitial keratitis benefit from the treatment of the underlying systemic infection.
