This protocol applies to patients with influenza who have not achieved adequate reduction in fever and influenza symptoms following an initial course of antiviral therapy, or who deteriorate after an initial response.
The first-line approach uses a single neuraminidase inhibitor — oseltamivir, zanamivir, or peramivir — with the treatment goal of reducing the duration of fever and influenza symptoms. When that goal is not met, or when the patient worsens after early improvement, escalation to the next line is indicated.
In patients who fail to improve or who deteriorate despite ongoing antiviral treatment, the next step involves investigating for bacterial coinfection of the respiratory tract and initiating empirical treatment for it, alongside continued antiviral therapy.
