Treatment of Community-Acquired Native Valve Infective Endocarditis Before Pathogen Identification

This protocol covers empiric antibiotic management for community-acquired native valve infective endocarditis (NVE) and for late prosthetic valve infective endocarditis (PVE) — defined as onset 12 months or more after valve surgery — in the period before a causative pathogen has been microbiologically identified.

The patient presents with infective endocarditis on a native valve acquired outside of a healthcare setting, or with prosthetic valve endocarditis classified as late-onset (≥12 months post-surgery). Empiric broad-spectrum therapy is required while blood culture and microbiological results are pending.

Empiric management relies on parenteral combination antibiotic therapy with ampicillin as the central agent, paired with a complementary second agent. Two regimen options are outlined in the full protocol, each with distinct companion drugs.
