Hyperemesis Gravidarum Not Responding to Ondansetron: Next-Line Management

This protocol covers the next management step for hyperemesis gravidarum when standard antiemetic therapy has been tried and the primary clinical goal — resolution of nausea and vomiting — has not been reached.

Previous Treatment — Target Not Achieved

Antiemetic therapy with Ondansetron — and, where reflux was a contributing factor, Famotidine or Omeprazole — did not achieve resolution of nausea and vomiting. Escalation to the next line of management is indicated.

This Protocol

Once antiemetic medications and IV hydration have not been sufficient, steroid-based therapy forms the basis of the next approach — with further options available in selected cases. The complete agent choices, sequencing, and clinical decision points are in the full protocol.

Clinical Target

Resolution of nausea and vomiting.
