Treatment of Hydatid Disease of Liver with CE2 or CE3b Cyst >5 cm

Clinical scenario

This protocol applies to patients with uncomplicated hepatic cystic echinococcosis whose cyst is classified as WHO‑IWGE type CE2 or CE3b and measures more than 5 cm in diameter — a presentation for which active intervention is indicated.


Treatment approach

For CE2 and CE3b cysts exceeding 5 cm, the recommended approach combines surgical intervention with antiparasitic pharmacotherapy — with the specific agents, perioperative schedule, and adjunctive measures for particular intraoperative circumstances detailed in the full protocol.

Treatment goals

Response is assessed by cyst changes at 3 months — including detachment of the parasitic layers from the outer cyst wall, size reduction, or stage modification. Complete response is evaluated no earlier than 12 months after treatment end.

  1. In patients with uncomplicated hepatic cyst types CE2 or CE3b > 5 cm, surgery combined with ALB is suggested.
  2. This can be open surgery (in tiers 2–4) or laparoscopy (in tiers 3–4).
  3. A lack of response is defined as an absence of cyst changes after 3 months of treatment (detachment of the parasitic layers from the outer cyst wall, size reduction or stage modification).
  4. Complete response should be evaluated not earlier than 12 months after treatment end.
