High-Risk Classical Hodgkin Lymphoma in Children and Adolescents (Age ≤18)

Clinical Scenario

This protocol addresses children and adolescents aged 18 years or younger diagnosed with classical Hodgkin lymphoma at high-risk presentation. High-risk is defined by any of the following stages:

Stage IIB with bulky disease Stage IIIB Stage IV

First-Line Treatment Approach

Treatment in this pediatric high-risk setting centres on dose-intensive combination chemotherapy regimens, with some protocols also incorporating radiation therapy. Several combination approaches are available for this population.

Treatment Goal

A key objective is achieving a rapid early response to initial chemotherapy, assessed through systematic early-treatment response monitoring. Patients who respond quickly are classified as rapid early responders (RER).

High-Risk Disease (stage IIB with bulk, stage IIIB, stage IV)
High-Risk Disease: BV plus DHAP, BV-AVE-PC, ABVE-PC, OEPA or OPPA plus COPP or COPDAC, BEACOPP (may be followed by COPP/ABV or ABVD)
A patient who responds quickly to initial treatment is considered a rapid early responder (RER).
