प्रारंभिक एंटीबायोटिक उपचार के बाद स्ट्रेप्टोकोकल मेनिनजाइटिस में सुधार न होना
यह प्रोटोकॉल उन रोगियों में Streptococcus pneumoniae के कारण बैक्टीरियल मेनिनजाइटिस को संबोधित करता है जिन्हें गंभीर एंटीबायोटिक (बीटा-लैक्टम) एलर्जी नहीं है और जो प्रारंभिक एंटीबायोटिक कोर्स पूरा करने के बाद नैदानिक सुधार प्राप्त नहीं कर सके हैं।
पिछला उपचार और विफलता की स्थिति
प्रथम-पंक्ति प्रबंधन में अंतःशिरा सेफ्ट्रिएक्सोन — या सेफोटैक्सिम जब सेफ्ट्रिएक्सोन प्रतिबंधित था — शामिल था। उस उपचार का लक्ष्य 10 दिनों के भीतर नैदानिक सुधार था। 10वें दिन तक उस लक्ष्य को प्राप्त न करना अगले कदम की ओर बढ़ने का संकेत है।
