HELLP सिंड्रोम का प्रथम-पंक्ति उपचार

HELLP सिंड्रोम महत्वपूर्ण मातृ रुग्णता और मृत्युदर से जुड़ा है, जिसके लिए त्वरित, संरचित प्रबंधन आवश्यक है। प्रसव की नियत तिथि से दूर स्थित रोगियों को नवजात शिशु और प्रसूति गहन देखभाल विशेषज्ञता वाले तृतीयक देखभाल केंद्र में उपचार की आवश्यकता होती है। प्रथम-पंक्ति प्रोटोकॉल प्रसव के समय निर्धारण, दौरे की रोकथाम, और तीव्र गंभीर उच्च रक्तचाप के प्रबंधन को प्राथमिकता देता है।

उपचार दृष्टिकोण

प्रबंधन मातृ स्थिरीकरण से शुरू होता है, उसके बाद प्रसव होता है, चाहे गर्भकालीन आयु कुछ भी हो। दौरा रोकथाम देखभाल का एक केंद्रीय घटक है। तीव्र गंभीर उच्च रक्तचाप के लिए शीघ्र उच्चरक्तचापरोधी चिकित्सा की आवश्यकता होती है। प्रसव के समय गर्भकालीन आयु पूर्ण प्रोटोकॉल में अतिरिक्त हस्तक्षेप निर्धारित करती है।

  1. Considering the serious nature of this entity, with increased rates of maternal morbidity and mortality, many authors have concluded that women with HELLP syndrome should be delivered regardless of their gestational age.
  2. Because the management of patients with HELLP syndrome requires the availability of neonatal and obstetric intensive care units and personnel with special expertise, patients with HELLP syndrome who are remote from term should receive care at a tertiary care center.
  3. Magnesium sulfate should be used for the prevention and treatment of seizures in women with gestational hypertension and preeclampsia with severe features or eclampsia.
  4. Antihypertensive treatment should be initiated expeditiously for acute-onset severe hypertension (systolic blood pressure of 160 mm Hg or more or diastolic blood pressure of 110 mm Hg or more, or both) that is confirmed as persistent (15 minutes or more).
  5. With supportive care alone, 90% of patients with HELLP syndrome will have platelet count more than 100,000 ×10&sup9;/L and reversed trend (decrease) in liver enzymes values within 7 days after delivery.
